Glitched out of reality
You go through a part of the wall which is actually floor and looks slightly different then the other parts, then it gives you a FAKE blue screen and you earn the achievement
Stuck in the multiverse: You learn that the universe is coded, and you want to break out of it, so you go to Morpheus from the Matrix, and you take the blue pill and forget everything about the universe.
Broke out of the multiverse: The same thing as the first one but you take the red pill and brake out
